Hi doctor,

I had unprotected sex with a girl. Ten days later, she texted me telling me she tested positive for chlamydia, so I called the university clinic, and they said they were not doing STI testing currently. Only covid testing, so I was prescribed the medicine straight away, which was tablet Azithromycin 250 mg taken all at once and was instructed to wait one week before having sex again. So after seven days, I had protected sex with a girl. The first time I ejaculated, everything felt fine. The second time it felt as if I could not ejaculate fully, and when I went to the toilet after it directly to pee, I could not pee. There was transparent fluid discharge, and it felt as if my urethra was blocked. There was also a burning sensation when this happened. Then, I called the university clinic and got in touch with a doctor the same day. He asked me if I had symptoms before, and I told him I did not, which was the case. But only after I had sex after seven days did I feel that way, and in the two upcoming days after that, when I masturbate, I ejaculate, then I go pee after it there is transparent discharge for a bit, but only a little. The doctor yesterday, when I called him, represcribed tabet Azithromycin 250 mg four tablets all at once, and I took it yesterday. He told me to wait for five days and monitor symptoms. Today, I masturbated, and there was some transparent discharge after I ejaculated and peed. What is going on? Do I still have chlamydia? Is this treatment working or not? One year ago, I had sex with a condom, and I felt some burning and itching. The doctor told me it was a UTI. Please help.

I understand your concern.

If we consider chlamydia, then a single dose of antibiotic is sufficient to treat the infection, and most patients recover. I think you are recovered from chlamydia, but pus-like discharge during the passing of urine could be due to a urinary tract infection. This should be evaluated properly. The infection can be in semen, too, though chances are rare. I would suggest you get a urine routine, microscopy and culture sensitivity of urine is needed. This will help in making a diagnosis. If there is UTI (urinary tract infection), this should be treated accordingly. I suggest you avoid sex till you recover and have plenty of water and citrus fruits.
