Hello doctor,

My wife has had a chronic cough. It used to happen early last year and just started back this year. After she climaxes, she begins to cough uncontrollably and gets shortness of breath. She coughs up a thick fatty substance mixed with pinkish fluid. As the substance sits, it begins to separate and coagulate. This coughing last for an hour or two. She wheezes for a few days and has chest and lower stomach pain. She has chest pain with a burning sensation. Kindly suggest.

Due to excess excitement, mild cough can be present. But she has dyspnea and cough with expectoration; hence the cause needs to be evaluated. The following workup needs to be done to rule out any underlying respiratory or cardiac causes.

  1. Auscultation.
  2. Spirometry.
  3. Sputum examination.
  4. Chest X-ray if required.
  5. ECG (electrocardiogram).

Based on the results of these tests, the cause can be identified and treated accordingly. Consult a pulmonologist for examination and the above workup.
