Hello doctor,

A few months back, I had to use the bathroom at a friends house, and it was dark. After I used the bathroom and stood up, I used a flashlight, liquid fecal matter had dried all around the bowl, and my penis directly contacted it. A couple of weeks after I had an onset of frequent urination and irritation in the penis after urinating. The doctor prescribed Bactrim DS for five days. On day four of taking the Bactrim, I still had slight symptoms, and after the last dose on day five, the symptoms went away. Everything was fine, and I had no signs for 80 days, and then the same symptoms returned. I tried taking the Bactrim again for five days, and it did nothing. I have been having the exact same symptoms that I had when the Bactrim worked, and they have been consistent.

I have also had some clear discharge from the urethra a few different times, including once during a bowel movement and some in the toilet after urinating. I have an appointment coming up in a few weeks, but I am trying to get some information, wondering if the first treatment made my symptoms go away but did not kill the bacteria completely, and now is it in the prostate? These two bouts of the same symptoms have to be related, would you agree? I have had three urinalysis, and they showed normal results and a digital rectal exam by an urgent care doctor, and he said it felt fine but maybe slightly enlarged. I had a CT scan without contrast which was clear. It did state pelvic renal cysts were present. Do these cysts cause these symptoms? I have not been sexually active.

Thanks for consulting us. You are absolutely right about the chronic prostatitis diagnosis. This condition is present with recurrent symptoms of burning urination and perineal pain or deep pain or discomfort in the pelvis. Usually, symptoms appear and then disappear for many months. Chronic prostatitis can be bacterial or abacterial (without bacteria), and urine for culture is always negative. The best test for diagnosis is by semen for culture, and then I can choose the suitable antibiotics for you. Treatment will require six weeks of the sensitive antibiotic according to the culture result.

Thanks for your reply. That is why I am confused because I do not have any pelvic or perineal pain. My symptoms are frequent urination, irritation in the penis after urinating, and clear discharge from the penis a couple of times, which also comes out in the urine. With these symptoms, do you think it is still prostatitis? Also, why did it go away the first time after only five days of Bactrim? Also, why did I not have an abnormal rectal exam?

Yes, it is chronic prostatitis. Symptoms can vary. Frequency and discharge are the main signs. It is long-term, recurrent, and negative cultures with normal radiology investigations. It can respond in five days, but for complete treatment, you need six weeks as the prostate have a capsule, and it will need a long term of antibiotics to penetrate the capsule completely. Maybe the doctor did not expect chronic prostatitis. It is difficult to diagnose, and many people will think it is a urinary tract infection.
