Hello doctor,

I am 37-years-old male with a height of 5.8 inches and a weight of 260 pounds. I was circumcised at birth. But now, my penis has all the characters of an uncircumcised penis. It produces smegma, odor, and sometimes yeast infections. I am embarrassed about it, and hence I have not asked this to a physician during an in-person appointment. I want to share some photos of my genitals so the doctor can see what I am referring to. I have questions about hygiene and care and whether it is possible to improve the appearance through any known medical procedure.

I have seen the pictures you sent (attachments removed to protect the patient’s identity). Yes, you are right; despite circumcision, you have got a lot of unrequired skin. So you have to repeat the surgery to get rid of extra skin, and it will take care of hygiene issues and appearance. In your case, laser circumcision can be done. It is a daycare procedure with good results.

My foreskin looks like shaft skin. I am worried that a second circumcision will result in the shortening of my penis. If I push on the pubic fat pad, my penis looks circumcised. Do you think I could get a pubic lift? I think that will leave the glans uncovered, which will help with the smell and smegma. Sorry for these questions about my penis, but I have no one to ask. I have attached a few photos to see what I mean by the shaft skin on my penis. And please let me know if a pubic lift will help.

Smegma is a typical collection of natural body secretions like oil and dead skin cells. So, it is normal to produce smegma in a single day. Yes, you are correct in observing the penis. When you compress suprapubic fat, your penis looks like a normally circumcised penis which does not require further intervention, but when compression to the suprapubic area is not applied, a lot of skin over glans double folds, and that is the reason for the collection of secretions also. My verdict is to get circumcision again for better results unless you are shedding those extra pounds.

I was wondering if the liposuction of the fat pad and a pubic lift would solve the problem. I am not sure if circumcision will help because it looks like shaft skin that is folding over. I think it is shaft skin because the skin is much thicker than typical foreskin, which means that the foreskin retracts, but it does not stay retracted unless I hold it. Also, I am considering foreskin restoration, which is a natural process of regenerating the foreskin. I feel that my penis is in between a circumcised and uncircumcised penis. If I circumcise again, I am afraid of shortening the penis. Do you have any thoughts or concerns about that? Have you ever seen a penis like mine? In your medical opinion, does my penis look uncircumcised or circumcised with a lot of extra skin? I have attached a few more photos. I am somewhat embarrassed to show my penis to a physician in person. This service helps tremendously. Any insight you can provide is helpful and appreciated.

Liposuction can help to some extent, not sure how much. Circumcision will help. Your penis is between a circumcised and an uncircumcised penis; that is why I think a complete circumcision will help you more. Yes, I have seen penis as you have, and completion of circumcision is the answer. My verdict is, if I had been at your place, I would have gone for completion circumcision as my first option. But looking at your concerns and psychology, I think you should go for liposuction first. If you are satisfied with the results, nothing else is needed; otherwise, get circumcision as the next step.

Good to connect with you. I found it helpful. I just wanted to follow up with some additional questions and thoughts. I believe in the message said by you. You said that it is normal to experience some itching and irritation occasionally because of the foreskin. I wanted to make sure whether it remained normal. I went camping over a weekend and could not get a proper shower. As a result, my glans and inner foreskin became more reddish or pink than usual, and my frenulum became firm (like a seed) and tender. Is that also a normal reaction with uncircumcised men? Whenever I get an itch or a minor irritation, Clotrimazole 2% vaginal cream seems to work. Do you have any other pharmaceutical recommendations for both minor and significant irritation (as described above)? I have attached photos of my frenulum and inner foreskin so you can see the areas that become red and irritated. I also used a marker to mark the foreskin so that you could see. If you look at the pictures and see the markings, would you still recommend circumcision? If so, do you still recommend an entire or minor (touch-up) circumcision procedure? I am going to make appointments for in-person consultations. A urologist would perform the circumcision, correct? or a general surgeon? Thanks so much for your time. I appreciate it.
