Patient's Query

Hello doctor,

I am 23-year-old female with 5’5 feet in height. I have had pain near my right collarbone for the past few days. Two days ago, the pain spread to my right arm, fingers, and sometimes the right side of my jaw. The pain does not subside with Tylenol and wakes me up at night. I am scared that this is a brain tumor.

Please help.

Hello,

Welcome to icliniq.com.

I would like to know some details about your symptoms:

1) What type of pain do you have near the right collar bone? Is it stretching/ heaviness/ tightening/ electric shock-like / burning/ something else?

2) What pain do you have in your right arm and fingers?

3) Is this pain persistent or intermittent?

4) Have you noticed any aggravating factors for this pain, like - coughing/ sneezing/ lifting weight/ neck movements/ something else?

5) Do you also have some tingling or burning in your fingers or arm?

6) Is this pain affecting your sleep?

7) Have you recently had any injury or fall or lifted a very heavy object?

8) Does pain increase on local touch or pressure over the neck or arm?

Regards.

Patient's Query

Hi doctor,

Thank you for the reply.

1) I would say the pain is burning/tightening.

2) Arm feels sore and my fingers feel numb.

3) Pain is persistent but seems to be subsiding today.

4) I have not noticed any aggravating factors.

5) No burning or tingling- just more like soreness.

6) It does affect my sleep, but I think stress is as well.

7) No, I have not had any injury, fall or lifted heavy objects.

8) If I touch my neck area where it hurts, it feels very sore.

I am sorry for number one. I did not mean to say burning, and I meant to say tightening and heaviness.

Hi,

Welcome back to icliniq.com.

As per your description, there may be the possibility of some muscle spasm in your right side neck area and may be associated with exiting nerve pressure in the cervical spine, causing radiating pain in the right arm. Therefore, I would suggest some investigations:

1) MRI (magnetic resonance imaging) cervical spine.

2) Nerve conduction study of both upper limbs.

3) Serum Vitamin b12.

4) Serum vitamin D3.

For treatment, I would suggest you try:

1) Tablet Tolperisone 150 mg twice daily for seven days.

2) Tablet Gabapentin 100 mg thrice daily for seven days.

Along with restricting neck movements by using a soft cervical collar. Avoid using a soft and high pillow while sleeping. I can review it after seven days with investigation reports.

Same symptoms don't mean you have the same problem. Consult a doctor now!
