Hi doctor,

I want to discuss conception.

It has been four months of trying with an online medical consultation platform and following their advice but not being able to conceive. I feel like there is nothing normal with my body. I have pain in my lower portion and on my back every time. I also feel a swelling inside, or maybe a cyst is developing, because, in my history, I have a cyst in my right ovary. Please help.

I thoroughly read your query and understand your concern.

I suggest you first have some changes in daily life habits, follow some healthy tips and apply a few precautions, do some investigations and then try with full belief to get conceived.

  • Have one vaginal swab to check for any infection, as pelvic infection can give you pain and some infection. The cause can be due to fertility issues.
  • Have Folic acid daily 0.4 mg.
  • After collection of swabs, have tablet Doxycycline 100 mg 12 hourly and tablet Metronidazole 400 mg eight hourly for seven days. Take these tablets after consulting your physician.
  • Have one transvaginal scan and collect a high vaginal swab report. Collect both reports and discuss them here or with a local gynecologist.
  • Have sex in the fertile period of the cycle. If the period cycle is for 28 days, then it would be from day 9 to day 13.
  • Have a healthy diet with less salt, sugar, oil, and spice and walk daily and do light exercise to have a normal BMI.
  • Participate in recreational activities.

Try applying all the above precautions and tips with a positive wish. Normally investigations are to be done after at least one year of unprotected sex or, in some places, after two years. If you are unable to conceive even after applying all the above advice, then do blood tests on day two of menses after at least six months (serum progesterone, insulin, testosterone, FSH (follicle-stimulating hormone), LH (luteinizing hormone) and prolactin).
