Hi doctor,

My baby's palms and soles of hands and feet turn black when crying loudly and breathing heavily. The baby cannot drink milk for an extended period and is constantly sweating. In addition, the infant wakes up unexpectedly from sleep due to breathing difficulty. The oxygen level is at 58 percent. The echocardiogram report detected congenital heart disease. The report's findings include DORV (TOF variety) - a large malaligned VSD measuring 9 mm with a bidirectional shunt. There is an aorta override of 80 percent with aorta-mitral discontinuity present. GDT (goal directed therapy) of 65mm Hg indicates severe infundibular and valvular pulmonary stenosis. MPA (microscopic poly angiitis) with confluent branch PA is hypoplastic. A small 2.4 mm PDA with an L-R shunt. the infant was also administered 10mg of Indever, mixing 1cc in 10cc of water, thrice a day along with a nasal drop for snoring. Good biventricular activity in the right aortic arch. What should I do now? Is it essential to place a PDA stent immediately? Can surgery be performed at this age?
