Hello doctor,

I am a 58-year-old male. I am 180 cm in height and weigh about 85 kg. I have frequent constipation issues. I am currently on Obimet SR 500, Glimisave M1, Pinom 40 mg, Pivasta 2 mg, and Clopilet 75 mg.

Would you please answer few of my following questions.

How frequently do you pass stools per week? Have you felt any blood in stools? Do you feel pain in the anal area while passing stools? Have you noticed anything coming out from the anal area while passing stools? Have you noticed any loss of weight? If yes then how much?

Any abdominal pain? Do you strain while defecating? Any feeling of incomplete evacuation after passing stools? Have you used any medication for constipation? Any change in drugs for diabetes and hypertension in the past six months? Do you have alternating diarrhea with constipation? Do you feel bloated?

I will start symptomatic treatment and some workup for constipation. Please feel free to ask anything and answer my questions.

Investigations to be done

CBC (complete blood count). HbA1C (glycated hemoglobin). TSH (thyroid-stimulating hormone). Serum calcium. Digital rectal examination.

Treatment plan

Ispaghol husk two teaspoons in a glass of water every night. Polyethylene glycol sachet in a glass of water every night for two weeks.

Preventive measures

Take plenty of water. Do 20-30 minutes walk daily.

Thank you doctor,

I pass stools almost daily and I do not have blood in stools. I feel anal itching sometimes while passing stools. I have not noticed anything coming out from the anal area while passing stools. I do not have any weight loss.

Very rarely I have abdominal pain and I feel strain while defecating and incomplete evacuation after passing stools. Sometimes I used OTC laxatives but very limited. I have not changed any drugs for diabetes and hypertension in the past six months. And I have alternating diarrhea with constipation and bloating.

I presume there are no alarm features in your symptoms. It may be simple irritable bowel syndrome (constipation predominant). Please start the following treatment and tell me about the response in two weeks time.

Treatment plan

Ispaghol husk two teaspoons in a glass of water every night. Polyethylene glycol sachet in a glass of water every night for two weeks. Tablet Levopraid (Levosulpiride) 25 mg thrice a day before meals for one month.

Preventive measures

Take plenty of water. Do 20-30 minutes walk daily.

Same symptoms don't mean you have the same problem. Consult a doctor now!
