Hi doctor,

My partner has been using contraceptive pills for three months now, but she always starts a day after the last day of her period. Every time we had sexual intercourse during the last three months, I always used condoms even though she was on the pills. In the last two months, she menstruated on the fifth and sixth pill-free days, respectively.

Last month, she menstruated for five days. The day after her periods stopped, she immediately started taking the pills again. She has been taking all the pills religiously on time.

But, this month we had unprotected sex on the ninth or tenth day of her pill, but I pulled it out of her. Either on the eighteenth or nineteenth day of her pills, we again had unprotected sex, and I had ejaculated inside her. Yesterday, on her third pill-free day, we did it again unprotected, but I did not go inside her.

Today is her fifth pill-free day, and she is not yet bleeding. She has been already experiencing pre-menstrual stress symptoms for a few days (cramps, headache, tender breasts), but today they seemed to have eased up a bit.

She had a sty so she took Tobramycin-Dexamethasone eye drops for a couple of days.

I know that the pills will be effective after seven days of taking them and will still be effective during her pill-free days. But based on the information I gave, is there any chance that she will get pregnant as she has been regularly taking the contraceptive pills?

I have gone through your history and understand your concerns.

As per your description, these symptoms could be due to hormonal imbalance. But, as she is taking the pills on regular basis there is almost no chance of pregnancy in her case. The contraceptive pills are almost 99 % effective.

Get the pelvic ultrasound done to see the endometrial thickness and any other sign of pregnancy.

Get the thyroid profile and hemoglobin test also done.

She can also go for a home urine pregnancy test to rule out pregnancy if she misses her period for seven days.
