Hi doctor,

I have pain for the past four weeks already. The pain is consistently sharp and takes my breath away, and stabbing. And with the pain, I have spasms all over my body. I can feel some pulsation inside my left upper side under my ribs and the pain starts with the cramps on my back going to the front with the contraction pain. My blood and urine are fine. I had done an MRI scan.

And the MRI says normal appearance of the liver. No focal lesions are seen. The gallbladder is normal. No intrahepatic biliary duct or common bile duct dilatation. I just would like to know what if no intrahepatic biliary duct or common bile duct dilatation is normal. Please help me to understand.

Inflammatory conditions like costochondritis must be ruled out. As of now, you can use analgesics and anti-inflammatory combination like Aceclofenac, Seratiopeptdase for symptomatic relief. If symptoms persist, it is better to consult a physician and get evaluated.

With these symptoms I end up six times in an emergency. Every time they gave me the medication and sent me home. I am allergic to Tramadol, Codeine, and Morphine. The GP told me the Pregabalin will relieve the pain but I have the pain and when I eat the pain is stronger.

Based on your symptoms and as all the investigations are negative, costochondritis will be a possible cause. Rarely conditions like fibromyalgia can also cause similar symptoms (less likely in your case).

The problem with this situation is that as there is no definite organic cause like nerve injury or mass lesion treatment will be supportive to relieve symptoms.

Anti-inflammatory pills like Seratiopeptdase or Trypsin, Chymotrypsin combination can provide symptomatic relief. You can also opt for newer techniques like ultrasonic and TENS (transcutaneous electric nerve stimulation) also. Do you have any tenderness while touching any part of ribs or sternum?

Thank you doctor,

I do not any tenderness while touching any part of ribs or sternum. I feel pain underneath my left side upper ribs. When the pain is 10 I have a spasm and all of my body looks like fits and maid me unconscious, my heart goes up and down makes me very worried and weak. What kind of doctor should I see? And I have a polyp on my gallbladder which was 5 mm.

Polyps has got nothing to do with it. Costochondritis will be the most possible differential diagnosis. You can continue analgesics, anti-inflammatory combination.

As I said earlier newer techniques like ultrasonic or TENS (transcutaneous electrical nerve stimulation) will be effective. You can consult a pain physician (or a physical medicine and rehabilitation specialist) and he will direct you accordingly.
