Patient's Query

Hi doctor,

My mother had undergone an angioplasty three months before. Her vitals, like sugar level, blood pressure, and all are fine. But she has developed a cough which comes after a slight tickling sensation in her mouth area, and the cough is a kind of relief for her. We did take her to the ENTs and MD doctors, but they say it is just an acidity. But their medicines are not helping. And she is on ACE inhibitors. Please help. What could be the reason behind this coughing?

Hi,

Welcome to icliniq.com.

If the cough has started after ACE (angiotensin-converting enzyme) inhibitor, I suggest she stop it and replace it with ARB (angiotensin receptor blockers) like Valsartan 80 mg at evening time. If the cough is chronic, then cause should be sought. Allergy, post-nasal drip, acid regurgitation can be some causes. Does she have post-nasal dripping or acid reflux, or sore brash? Does she have asthma or any allergy? Did this cough start after starting the ACE inhibitors?

Patient's Query

Hi doctor,

Thanks for the reply. She does not have nasal dripping and no sensation of acid reflux. She does not have asthma or an allergy. It is hard to guess whether the cough started after ACE inhibitors because it started after her second visit. And after the second visit, Concor 5 mg is the only addition to her medicines. So now she is taking both Concor 5 mg and Ramipril.

Hi,

Welcome back to icliniq.com.

The first step is to replace ACE inhibitors with ARB. So I suggest she replace Ramipril with Valsartan 80 mg. And replace Concor with Nebivolol 5 mg. Does she have shortness of breath or breathlessness while walking or while lying on the bed? Is the cough dry or with sputum?
