Hi doctor,

I am a 52-year-old female. I do not have any health issues. Initially, I took the first dose of Sinopharm. I got severe diarrhea and stomach cramps. I was given Metronidazole. My intestinal issues got cleared up. But when I waked up in the night, I had an average heart racing two times. It caused trembling, lightheadedness and dizziness.

Thank you for the query. I can understand your concern. According to your statement, you are suffering from heart racing, trembling, lightheadedness and dizziness after getting the first dose of Covid vaccine called Sinopharm. Like other medications, this Covid vaccine has some side effects of its own too. Very common side effects of Covid vaccine that may affect more than one in ten people are pain, swelling, redness, or irritation at the site of injection administration, tiredness, headache, muscle pain, diarrhea, joint pain, and fever. These side effects usually go away within three days after getting Covid vaccine.

The uncommon side effects of this medication that may affect up to one in 100 people are dizziness or sleepiness, decreased appetite, or abdominal pain. The rare side effects of Covid vaccine which may affect up to one in 1,000 people are inflammation of the heart like myocarditis or pericarditis, blood clotting phenomena, a sudden feeling of cold with shivering or shaking accompanied by a rise in temperature with sweating or headache. So, you may suffer from Sinopharm induced common or uncommon, or rare side effects collectively. If any of these side effects persist for four days to four weeks, then you should visit your doctor physically for undergoing some investigations and further treatment purposes.

Again, electrolytes imbalance that may result from severe diarrhea after getting Covid vaccine can also cause these types of clinical features. Again, as covid vaccine takes few weeks to develop immunity or antibodies against coronavirus, so vaccinated person can get affected by covid-19 virus during this period of time or just after getting covid vaccine.

If you suffer from such type of clinical features for more than four days, then you need to undergo some investigations like ECHO (echocardiogram), ECG (electrocardiogram), D-dimer test, serum electrolytes, and RT-PCR (reverse transcription-polymerase chain reaction) for Covid-19 virus (Coronavirus disease 2019) to exclude the cause. Treatment depends on the cause. So, the underlying cause has to be found out by doing investigations that I have already mentioned and then treat them accordingly. Take care.
