Hi doctor,

I am a mother of a 21-month-old baby, and post his birth, my doctor advised me to take Dronis 20 as a contraceptive. I was taking it regularly. However, last month, I did not have my period properly. Last month it was just spotting for five to six days, and then I started retaking Dronis. However, this month, it has been more than a week of completing my 24 days course, but I still have not yet had my periods. The last intercourse we had was two months before. Please advise.

I understand your concern. May I know from when you are on Dronis 20 (Ethinyl estradiol and Drospirenone)? Also, have you taken a pregnancy test (there are possibilities of pregnancy even when on the pill)? This is to rule out any possibility of pregnancy (as you have a history of intercourse two months before with only minimal spotting and delayed period this month). If negative, do not take the Dronis tablets (even if minimal spotting occurs) to cause withdrawal bleeding, and then Dronis can be restarted. Kindly take the pregnancy test and revert with the report, and if negative, I shall explain your next steps and reasons for the delay.

Spotting while on Dronis 20 is a side effect known as breakthrough spotting because of low estrogen levels, which occurs a few days before the proper period. Hence, taking Dronis on day 2 of this spotting or breakthrough bleeding (which you thought was your period) will cause an imbalance in hormones and delay your natural cycle even when you take Dronis properly. So this explains the delay of the period in the absence of a pregnancy. Did you take the pregnancy test? If negative, then I suggest Progesterone (Regestrone 10 mg) twice daily in the morning and at night for five days, and once you stop, you will get your periods within three to seven days. Once you start your period, you can go back to Dronis 20 on day two. I hope this was useful. Kindly do the pregnancy test before going forward with any treatment.
