Why do I get menstrual cramps with my late period?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am 26 years old, 132.2 pounds, and 5 feet 5 inches tall. My period is usually the same time every month, but this month it was a day late.

Now it is here, but there is nothing on my pad; I only see blood when I wipe, and it seems more like spotting. I am also cramping badly.

Kindly help.

Thank you.

Hello,

Welcome to icliniq.com.

I understand your concern.

A light or delayed period with only spotting and severe cramping can happen for several reasons:

  1. Hormonal fluctuations: Stress, diet changes, or routine disruptions can affect your cycle, causing a lighter or delayed period.

  2. Early pregnancy: If you have been sexually active, implantation bleeding or an early pregnancy issue (like a miscarriage) could cause light bleeding and cramping. A pregnancy test can help rule this out.

  3. Anovulatory cycle: Sometimes, ovulation does not happen, leading to a lighter or irregular period.

  4. Uterine issues: Conditions like fibroids, polyps, or endometriosis can cause cramping and irregular bleeding.

  5. Birth control or medication effects: Starting or changing hormonal contraception can affect your period.

If your cramps are severe or the spotting continues without a normal flow, it is a good idea to see a doctor to check for any underlying issues. If you think you might be pregnant, taking a test would be a good first step.

I hope you are satisfied with my answer. For further queries, you can consult me at iCliniq.

Thank you.
