Hello doctor,

I am not getting periods for the last three months. I have with me some blood tests and USG pelvis scan. The report of that scan is normal but in the blood test, I found my free testosterone level has increased to 1.04 nq/dL and prolactin is 33.2 ng/ml. Please tell me if this is a serious problem.

I have seen your reports (attachment removed to protect patient identity). Free testosterone is very slightly increased but serum prolactin is remarkably increased and that can reduce or disturb cycles and even stop these. Are you on any treatment or hormonal intake or any symptoms of a headache or secretions from the breast? If any breast secretions are present, that is also a sign of excessive prolactin levels. Antidepressant tablets can increase prolactin or a few other treatments also can stimulate prolactin secretions or some brain mass can also increase prolactin secretions.

It is better to consult an endocrinologist. After a detailed history and examination, you might get advised to do a CT/MRI of the brain to exclude any brain space occupying lesion that can stimulate excessive secretion of prolactin. You might have to take treatment to reduce prolactin but only after a complete investigation has been done to find the exact cause. Do not stress about it. Do yoga, exercise, and other recreational activities. Discuss your detailed history with an endocrinologist as well. I hope after the radiological investigations of your brain, you receive appropriate treatment and have regular cycles.
