Patient's Query

Hello doctor,

My upper premolar on the right is not crowned after RCT. There is no first molar, and the second molar is neither crowned yet. There is a space between the lower first molar and second molar teeth where the food particles are getting stuck. So I want to know whether all the decayed first molars be extracted for braces instead of extracting the other three healthy premolars? Or can braces be decided later, even after reducing the upper premolar and second molar size for placing the bridge there?

Note that the teeth are only protruding a little and crowded. The local orthodontist and dentist told me to think well before crowning, but my guardian suggested to place the bridge. Will the braces be more painful if molar teeth are extracted instead of premolars? I hope you will answer all my queries soon as the second upper molar has a crack to be fixed soon.

Hello,

Welcome to icliniq.com.

I have gone through your query. I can understand your concern. Let me tell you, as per the ideal case or situation when one has to decide orthodontic extraction, then premolars are considered.

There is a reason behind this. As molars are pillars in our dentition (lay terms), major part of our chewing is carried out with molars' help. Moreover, molars have more roots and more surface area than premolars, so that will be more difficult to move the anterior teeth back if they are protruded (at the time of ortho treatment).

But in some cases, when there is a compromised condition, depending on the case one can change the plan of extraction of teeth. That can be suggested only after clinical examination. I want to suggest you consult an orthodontist and get a treatment plan before placing a bridge or reducing the bridge's teeth.

As per your details, it seems your case is interdisciplinary where more than one specialist needs to be consulted. So it is advisable to consult both prosthodontist and orthodontist regarding treatment plan for replacing and aligning teeth.

If you upload images of your concern area and arch, you can discuss the appropriate treatment plan for your case.

Same symptoms don't mean you have the same problem. Consult a doctor now!
