Hi doctor,

I am about to take my standard progressive metrics (SPM) for another two weeks, but now I feel so tired and dying about my life. I feel terrible; everything around me is so messed up, I do not have friends, and I am always doing my things at home. I am not socializing, and I feel like I am the one who always makes a mistake and trouble, like everyone is leaving me because I cannot even handle myself; sometimes, I lose my mind. I am doing the things I cannot get rid of it. A month ago, I always cried and ate Panadol every night to relax my mind. Sometimes also I do not want to eat and lose my weight. I feel insecure if I get fat and do not like my appearance. I have someone I love, my second mom, but my second mom passed away ten years ago, and I still cannot accept that my mom has passed away. I still need her beside me. I am so tired. I am dying. I feel useless. I am not worth it. I do not have anyone to share my problems. I do not trust anyone. And also, I am so lazy to go to school because I am not interested. I lost motivation a few years ago until now. I feel like death is the only way to solve my problems. Please help.

I can understand your situation. You may have major depressive disorder due to underlying stress. Unfortunately, there is non-pharmacological management for your depression. However, it would help if you tried following the guidelines below to deal with it. It will help you.

  1. You should exercise 30 to 45 minutes daily, like at least going for a walk. Joining the gym and swimming will boost confidence.
  2. You should do meditation for 20 minutes daily, sit in one place in a comfortable position, and try to concentrate on your normal breathing, do not stop or force yourself to stop your thoughts; just let it be. It will help you to relax.
  3. You should eat healthy home-cooked foods and avoid junk foods.
  4. You must drink enough water.
  5. You should spend quality time with friends and family daily or do video calls if you are living away from your family. You have to socialize and learn to socialize.
  6. It would help if you spent time on creative hobbies like reading good books, writing journals, painting, drawing, and learning new skills, language, or musical instruments.
  7. You should take enough seven to eight hours of sleep daily.
  8. It would be best if you avoided tobacco or alcohol, or any substance abuse.
  9. Live your life, do not just spend it. Be bold and confident. If you cannot share your thoughts with anyone, you can write them down in a diary. You have to push yourself to come out of this. If you still have symptoms that persist after following the advice for four to six weeks, then you need to start medications for a short duration. Please, let me know if you want some help.
