Hello doctor,

About a month ago, a hen scratched my hand. I do not know if there is a connection, but I got a rash. I used Triderm against fungus infection, and the rash almost disappeared. After that, I got a rash on other parts of the body. The doctor said the skin was dry, and I have dermatitis. He prescribed Urea body milk 10%. I have been using it for more than five days, but there is no result from this treatment.

Use Fucidin cream twice daily in the morning and noon for seven days, Miconazole ointment or gel at night for 15 days. Take tablet Levocetirizine 5 mg at night after dinner for seven days. Avoid scratching or rubbing lesions as it may cause secondary bacterial infection. It is dermatitis with secondary bacterial infection. Do you have a history of atopy or allergies? Do you have sensitive skin, or dry scaly skin? Is there any oozing lesions?

I think I have dry skin (face, hands), but it has not been examined. My skin is not scaly in general. Maybe my face is dry. However, I never had a significant rash before. I have allergies to some odors. I do not have oozing lesions. When I take a shower, the rashes get darker. They are not itchy at all (nor scaly), so I am not scratching, but I was using the cream prescribed by my doctor all over the body. Might this be the reason for the spread? Also, how safe is taking a shower? Will, it further spread the infection? Which soap would you recommend? Lastly, is the medicine you have indicated safe to use without consulting with a dermatologist in-person?

Yes, topical medication is safe to use. Apply Fucidin cream twice daily for seven days and Micogel (Miconazole nitrate) at night for 15 days. It is xerosis (dry skin), according to your symptoms mentioned above. Bathing too frequently, use of harsh soap, hot water showers can cause xerosis. Inadequate water intake, low humidity, dry winters, prolong exposure to the sun, A.C, central heat can also cause dry skin. Certain drugs can also cause dry skin. Stay hydrated. Moisturize the skin daily with Cetaphil moisturizing lotion immediately after the bath. Eat a diet rich in Zinc, Vitamin E, Omega 3 fatty acid. Avoid rubbing of the skin. Gently pat dry skin after a bath with a towel. Use olive oil and almond oil to soothe the skin. Also, avoid soap, detergents, perfume containing drying agent, alcohol, etc. You can use a mild soap like Dove, Cetaphil soap, or Pears for the bath. Use photostable sunscreen daily at 9 AM, 12, and 3 PM.
