Hi doctor,

I am a 64-year-old diabetic patient. I have diabetes for the last 12 years, and presently my FBS level is between 115 to 130 mg/dL. My last HbA1c was 7.1%. Currently, I am taking regular medicines named Glizid-MV one tablet before breakfast and Gliptin 20 mg before dinner. I am also taking Diavit plus and B-Fit-CD, and Seacod capsule as supplements. Can you suggest a new medication for me to improve my insulin sensitivity? I am taking a regular vegetarian diet and doing regular exercise.

I understand your concerns. If I am not wrong, the FBS (fasting blood sugar) is worrying you. Considering your age, I would say the sugars are optimally controlled on the medications you are currently on. I would not suggest a change in medications at this time. Your HbA1c (glycated hemoglobin) is optimal for your age. If we try for stringent control, it may lead to low sugars and complications. Kindly continue the same medications for two to three months and can be revisited by looking at HbA1c levels. The fasting and PPBS (postprandial blood sugar) for three consecutive days (day1 pre and post-dinner, day two pre and post-lunch, and day three, pre and post-dinner) sugar reading will help titrate the medications. This can be done at the same time as HbA1c. Do not worry but keep following a healthy diet, and adequate exercise appropriate for your age should be fine. Brisk walk and yoga can increase insulin sensitivity naturally. I hope this helps. Take care.
