Hi doctor,

My younger brother, of age 37-year, for the past few weeks, was observing weight loss and dullness of skin color. So today, he went for a blood test (fasting sugar, PP, CBC, RFT, LFT, TSH, and lipid profile). There is no complaint about health issues. His report is attached for your kind review and advice. Kindly give your opinion.

His blood sugars, thyroid profile, and triglycerides are deranged. The rest of the report is fine (attachments removed to protect the patient's identity). His symptoms are related to elevated blood sugar. He needs to be started on medications for sugar urgently. However, we can wait and see the effect of lifestyle modifications on other derangements. So it is important to get medications prescribed from a local physician. He should also be engaged in a healthy lifestyle as soon as possible. He should avoid sugars, sweets strictly and should also reduce the consumption of oily products. He should engage in regular physical activities for at least 30 minutes a day. So his diabetes needs to be treated on an urgent basis, and it is better to get medications like Metformin, etc., prescribed from local MD (doctor of medicine) physician or endocrinologist as it will be helpful in regular follow up. Also, he should buy a glucometer for home sugar monitoring as well, as he will need to monitor sugar atleast till sugars are well controlled. For a deranged thyroid profile, he may wait for medications and should repeat the thyroid profile after three to four months. I hope this was helpful.

As advised by you, my brother consulted a physician and started taking medicine as mentioned below. I am taking Glimipride .5 with Metformin 1000 mg - one tablet daily, Vilodagliptine 50 - one tablet daily, Olmesartan 20 - one tablet daily. Along with the medicine, he did a daily workout, controlled food habits, and followed some natural remedies like taking fenugreek seeds. After taking medication for ten days, he did a test to know his blood sugar and the result came back fine (attached). Following are my queries. After this test result, we need your advice on whether he can reduce the medicine dosage or, if not, for how long he should continue taking this dose of medicine? He has been having hypertension for seven years, and just now ( ten days before), he has started taking medicine for controlling blood pressure. Before taking medication, his blood pressure was 150/96 mmHg, and after ten days, his blood pressure was 132/76 mmHg. Would you suggest whether he should continue taking his medicine to control blood pressure?

His sugar level is good now. Both diabetes and hypertension are chronic problems and do not have a permanent cure. So medications need to be continued on a long-term basis. If medications are stopped now, sugar and blood pressure will rise again. However, with a healthy lifestyle and regular exercise, his requirement of medicine will decrease. So he needs to monitor sugar and blood pressure frequently. Get him sugar tested again after a week especially fasting, and if it stays in this range, then Glimepiride may be stopped. He should always carry some sweets with him, and whenever he experiences any symptoms like nervousness, sweating, and uneasiness, it may be due to low sugar, and he should consume sweets. For hypertension, he should continue with Olmesartan, and if blood pressure goes below 110 mmHg systolic, we need to reduce the dose. All the best.
