Can microvascular cranial nerve palsy cause drooping eyelids and blurred or double vision?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I am a 64-year-old woman who has had issues in the left eye for the past two to three days. I suffer from blurry vision and double vision. I was admitted to the ICU for elevated BP levels at 240 mmHg, which has now reduced to 140 mmHg. My left eyelid is now shut off, and I have no control over it. Also, I feel weakness on the right side of my body and experience pain while moving my right arm. A few people in my family say that I am acting, and since I have high BP, I should relax. I researched about it on the internet, and from that, I feel I may have microvascular cranial nerve palsy. If that is the case, what is the best treatment for it? And what procedures should I undergo to get control of my eye and feel better and healthy again? Please help.

Hi,

Welcome to icliniq.com.

I went through your query and understood your concern.

Yes, it is a case of microvascular infarction leading to complete ptosis and ophthalmoplegia. It is advisable to wait and watch as recovery is expected in the first three months of the appearance of symptoms. If not, a course of steroids can be given, but first the condition needs to be observed. So I suggest you to keep a check on your blood pressure and sugar levels. Also, I suggest you to keep using an eye lubricant like Carboxymethylcellulose eye drops and ointment at night. In addition, I suggest you to undergo an MRI (magnetic resonance imaging) of the orbit to rule out any mass or lesion. I hope this helps you out. Take care.
