I am a 34-year-old woman experiencing sudden twisting movements in my neck and shoulders. It is affecting my posture and making it difficult to work. My neurologist said it is a type of dystonia and suggested botulinum toxin injections. I wanted to know:
How often will I need these injections?
Are there any exercises or medications that can help as well?
Can stress or my menstrual cycle make the symptoms worse?

Your symptoms suggest cervical dystonia (neck dystonia), a condition in which certain neck and shoulder muscles contract involuntarily, causing twisting or abnormal posture. It is not dangerous, but it can be painful and may interfere with daily activities.
1. Botulinum toxin (Botox) injections:
This is the most effective treatment for your focal dystonia (a type of dystonia affecting one area of the body). The toxin relaxes overactive muscles by blocking their abnormal nerve signals.
Injections are usually given every three to four months.
Relief generally begins within five to 10 days, reaches its peak at about three to four weeks, and then it gradually fades before the next session.
When given regularly, it helps the patient to maintain their proper posture and reduces muscle pain and stiffness.
2. Other medications:
In some cases, oral medications such as Trihexyphenidyl (a muscle relaxant), Baclofen (a muscle relaxant and antispasmodic), or Clonazepam (an anti-anxiety and muscle-relaxing medicine) may be added for your additional symptom control. These medicines can sometimes cause drowsiness or dryness of the mouth.
3. Physiotherapy and exercises:
Gentle stretching and posture correction exercises can reduce your stiffness and improve neck movement.
Certain sensory tricks, such as lightly touching the chin or cheek, may temporarily relieve your abnormal pulling of muscles.
Heat therapy and relaxation techniques can also help ease your muscle tightness.
4. Triggers and progression:
Stress, fatigue, and hormonal changes around your menstruation may temporarily worsen symptoms.
Over time, cervical dystonia usually stabilizes rather than progressively worsens. With regular treatment, most patients can lead active and normal lives.
Botulinum toxin injections every few months, combined with relaxation, posture exercises, and stress control, help keep your symptoms well-managed. Early and consistent care is important to maintain neck movement and comfort in the long term.
