Hello doctor,

I am experiencing a headache along with congestion, confusion, and ear pain as if there is pressure in my ears. I am unsure if this might be an allergy such as sinusitis since I do not have many other symptoms beyond what have mentioned. Occasionally, I have a dry throat, but I think that is due to not drinking enough fluids. I had similar symptoms a few weeks ago, including a stuffy nose, and initially felt better, but the issue has returned. I should mention that the environmental conditions here are quite dry, and cold air is prevalent.

There has been very little rain recently, and the area has high levels of pollen and pollution. I saw a doctor in the early stages of this problem, and she prescribed an antihistamine (Loratadine). However, I only took it for one day because I felt anxious and experienced panic attacks, which I believe were side effects of the medication.

Currently, I am relying on home remedies, such as steam inhalation and using a warm cloth on my ears to alleviate the ear pain. I urgently need relief, as this issue is affecting my productivity and hindering my ability to think and work efficiently.

Based on your description, it sounds like you may have an ear infection. In the initial stages, ear infections often present with severe ear pain, a blocked sensation in the ear, decreased hearing, and sometimes fever. This typically occurs alongside a severe cold or sinusitis. If the infection is severe enough to cause a rupture of the eardrum, you might notice a slight discharge of blood or pus from the ear, which may temporarily relieve the earache. For an ear infection, you will need antibiotics, which can be prescribed after an examination by an ENT (ear, nose, and throat) specialist.

I suggest the following medications to help alleviate your symptoms. Please consult your doctor to discuss these recommendations and obtain their consent before starting any medication:

  1. Diclofenac tablets: Take one tablet after food, twice a day.
  2. Oxymetazoline hydrochloride nasal drops: Use three times a day for a maximum of five days. Prolonged use may worsen your symptoms.
  3. Antihistamine tablet: Choose either Fexofenadine or Levocetirizine and take one tablet at night.
  4. If you are prone to acidity, consider taking the Omeprazole capsule once in the morning on an empty stomach, as Diclofenac may cause gastrointestinal side effects.

In addition to these medications, you should do steam inhalation three times a day and drink plenty of warm fluids.
