Hi doctor,

My son suffers from an ear infection for two weeks. It causes pain to him. He does not have any weeping fluid and he is taking antibiotics. He does not have a fever and the eardrum appears to be in turquoise color with blisters. It seems to be pulsating. We went to the hospital three times and we are unable to manage the pain. He is an epileptic patient and he had several episodes. I have sent you some images.

Your photos seem to be hazy (attachment removed to protect patient identity). But it is fine. I can see some accumulation of fluids behind the eardrum. Please mention the antibiotics and analgesics taken. As he is an epileptic patient, there are some prohibited drugs for your child. The consulting neurologist needs to inform you. If you know them, then please mention them.

He took lots of oral amoxicillin and ear drop antibiotics. I do not know any prohibited drugs for epilepsy. We are using over-the-counter medications. I have attached the photo again. He has pus inside it for ten days. He has a clear blister and the ear drum is pulsating now. He has autism and language delay and so he struggles to verbalize things.

I can able to see the blisters now. The pus that filled the ear canal got faded out. This means that the eardrum has become perforated. With the help of Amoxicillin and antibiotic ear drops, this perforation got healed with a residual inflammation in the middle ear behind the eardrum. I suggest you have a change in antibiotics. Instead of Amoxicillin, you can take Zithrone 200 (Azithromycin) suspension. It is more powerful against a wide range of microorganisms. For those having amoxicillin resistance, the dose of Zithrone is 5 ml and it should be taken once daily for five days. You can also give Brufen syrup 5 ml every eight hours. It seems to have analgesic and anti-inflammatory properties. Use it for a week. You can also use any mucolytic syrups or sachets 5 ml every eight hours for a week. You can also use nasal decongestant drops twice daily for five days.
