Hello doctor.

My 5-year-old son's skin in the groin, on the shoulders, legs, and back of the knees has raised, red, and itchy patches. There have been flare-ups for the past six months. I have tried different moisturizers and diets; however, the severity has not reduced. He also had hay fever, which made his eyes swelled up and itchy. He has been using an eye spray and antihistamines for hay fever. The skin condition has become severe and affects the groin area the most. Also, he complains that the affected areas hurt the most when he gets into a bath. He is currently using a cream for eczema and used a steroid-based cream in the past.

Kindly advise.

I understand your concern. I have seen the attached photographs (attachments removed to protect the patient's identity). The child is having issues in the sweat-bearing areas of the body. Also, these rashes seem to be infected. I suggest the below treatment plan:

1. Powder Abzorb (Clotrimazole) to be applied to the affected areas after bathing and drying thoroughly.

2. Ointment T bact (Mupirocin) to be applied to the affected area at night for two weeks.

3. Frequent change of clothes as well as loose clothing to reduce the amount of sweat coming in contact with these areas.

Moisturizers should be avoided during the above treatment.
