What is the reason for the sudden increase in bilirubin level in two consecutive tests?

Patient's Query

Hi doctor,

I am 27 years old, and I had two blood tests. The first one was high in bilirubin levels in the liver. So, I was requested to have a second one, and the results came back as even higher. Doctors are saying they still have not had the results back. And are saying they have not had results still and cannot get me in to see doctors. I am feeling really low, and my head does not feel right. Yesterday also had runs and belly pains. I also have jaundice. I am currently working on the night shift. I need professional advice until I can get into the doctor's. Thanks.

Hello sir,

Welcome to icliniq.com.

I read your query. Sorry for the discomfort you are having.

Biochemical increase in bilirubin level above the upper limit of the normal range is clinically called jaundice.

You mentioned symptoms of stomach pain and jaundice.

  • Since how many days are you having these symptoms?
  • Do you have any nausea or vomiting?
  • Do you have a lack of appetite or weakness?
  • Do you have any fever?

It would be more helpful to guide you if you could attach the first report to see bilirubin levels.

What about the other enzyme levels of the liver function test?

Kindly attach the report or write the value of various parameters tested to guide you better.

Thanks.

Patient's Query

Hi doctor,

Thank you for your reply.

The symptoms have been there for the past four days now. The level of bilirubin was 37 on the first test and 45 on the second test. I have had jaundice for the last five days and have no nausea or vomiting. My appetite is normal, and I do not have any fever. I just feel low and run down now. Yesterday had runs and belly pain. The rest of the test came back normal.

Hi,

Welcome back to icliniq.com.

Thanks for your reply.

The first test had bilirubin levels of 37. There is a very acute rise in total bilirubin levels. The normal range of Total bilirubin level is between 0.3 to 1.2 mg/dl. Your level is 37 and now 45, maybe in a different unit, I hope so.

You mention rest of the liver function test is normal. Actually, total bilirubin in our body consists of direct and indirect bilirubin components.

When there is an isolated increase in total bilirubin levels only with other liver enzymes being normal, then there is the narrow differential diagnosis of its cause.

The clinician will then check which component, either direct or indirect, bilirubin level is increased to diagnose the cause of it.

Isolated direct and indirect hyperbilirubinemia has a different causes.

You should see the clinician with the first report itself as the value is very high (if its unit is mg/dl) to get proper evaluation and workup of the cause and diagnosis.

I hope you find this helpful.

Thanks.
