Hello doctor,

I am taking tablet Ovral-L for the last five months. My previous course of medication started in the middle of the last month and continued to the first week of this month. I got my periods ten days ago. I started the next course on the second day of my period.

I have had protected intercourse on the eighth day of my cycle and took an I-pill within five hours. I am continuing my Ovral-L also. Is there any chance of pregnancy still? Will I have irregular periods because of this?

Firstly as you were on tablet Ovral-L (Ethinyl Estradiol and Levonorgestrel) from the second day of your period, there is no need for an I-pill (Levonorgestrel). I-pill would cause a change in progesterone levels and initiate a withdrawal in the next few days. You may experience some spotting to mild bleeding as you are continuing the Ovral-L pack. The chances of pregnancy are minimal.

Do not take an emergency contraceptive pill if you are on a regular course of low-dose oral contraceptive pills.

Because you had your periods ten days ago, you would be on the eighth day of your cycle when you had intercourse. It is your safe period, and there are no chances of pregnancy. Moreover, you are taking tablet Ovral-L since the second day of your cycle. Hence, the protection established, avoiding the release of follicles and therefore avoiding ovulation, so no pregnancy.

In addition, you also took an emergency contraceptive pill, which is like triple protection.

So apparently, there are no chances of pregnancy; however, if you wish to be sure, you may get a serum beta hcg (human chorionic gonadotropin) test after a week to get conclusive results. You can continue Ovral-L on time to keep the protection for the cycle.

I have a low LH value now. The value is less than 1.5 IU/L.

Is it an indication of pregnancy?

LH (luteinizing hormone) is low as you are on tablet Ovral-L and the emergency oral contraceptive pill. Also, the value is as low as you are in the follicular phase. LH is not a marker of pregnancy. You should see if you have a withdrawal bleed in the next three to five days.

I have attached my progesterone report. I took an I-pill and continued taking my Ovral pills. Am I having low progesterone still? Did it not get absorbed? I thought that the value will become higher when I continue taking Ovral pills and I-pill. But it is too low. I am worried. Did it not get absorbed? I took I-pill on the eighth day of my cycle and my test was done on the tenth day of my cycle. Do Ovral and I-pill work in me? I am scared as I thought that taking I-pill will increase my progesterone a lot. But it did not help me and so I am worried. What are the chances of pregnancy?

The levels have expected changes after 48 hours of I-pill intake. The value of progesterone becomes high only on the next six hours post intake and then it gets reduced rapidly. So the levels in the follicular phase confirm that there was no ovulation apparently assuming that I-pill and Ovral L would have failed to protect. The progesterone is made post ovulation by the remnant follicular corpus luteum which confirms that it has not formed due to documented low progesterone levels, so there was no ovum present to fertilize. Sperms live only for 72 hours in the genital tract. As there was no ovulation involved due to contraceptive pills, so there would have been no fertilization and hence no chances of pregnancy. I hope that the physiology is clear. So be relaxed. You may or may not bleed or spot as Ovral l is continued so reliance on withdrawal bleed is questionable. Just to keep your anxiety at bay you may get a serum beta hCG test done to check for any pregnancy. Regards.
