Hi doctor,

I have been suffering from obesity. My weight is 103 kg, and my height is 167 cm. My period is always irregular because of my weight. Today I did an ultrasound, and the result is attached. So I need a second opinion. The doctor gave me Yaz, and I want to know if it is effective in treating the cyst. Also, I have an endometrial polyp. Is it serious? Is there a medicine for it?

I went through your query and understood your concern. The two main concerns are

  1. Subserous myoma with slightly thickening myometrium.
  2. Polycystic ovaries.
  3. As you are on the heavier side of weight gain, it is advisable to lose weight (the ideal weight for you is 65 kg). It would be best if you consult a dietician and do some exercises for at least 30 to 40 minutes per day. Yaz ((drospirenone and ethinyl estradiol tablets) medicine will help in the regularisation of your periods and treat PCOS (poly cystic ovarian syndrome) to some extent, but weight loss is substantial. The subserous myoma would not cause many problems now but may interfere with pregnancy. So if you want to get pregnant anytime soon, you need to remove the myoma/ polyp from a gynecologist.

    The para tubal cyst is a benign finding. So do not get stressed about it. Follow a healthy diet rich in protein, exercise regularly, and focus on a good sleep pattern.

How can the para tubal cyst be treated? Can it shrink or dissolve? If I undergo surgery, for example, can the cyst and myoma be removed together?

Paratubal cysts are benign findings. They do not require any specific treatment. They come and go on their own. Yaz is for the treatment of PCOS. If you want to remove the cyst and myoma together, you can undergo hystero laparoscopy in the same setting.

Also, I have a problem with being so emotional and sensitive. I feel sad and alone, and nobody, anything, can make me cry. I’m not like that, though. So I think maybe it’s because the birth control. I was taking Yasmine 4 months ago, and now Yaz. I cannot hold my tears, and it isn’t easy to control myself. Even when I’m writing to you now, I feel tears coming. It’s weird. I have some personal issues, nothing serious, but I would’ve never cried like that as if someone dear to me died. I don’t know what to do. I don’t have an option, and I must take Yaz for three months.

Some women taking oral contraceptive pills feel anxious and face mood swings, but it is not very prevalent. It would not make you feel like you cannot control your tears. If Yasmin (Drospirenone and Ethinyl Estradiol ) was affecting you, do not take Yaz. They are the same composition.

I suggest you take Diane 35 (Cyproterone and Ethinyl Estradiol) pills rather than Yaz. Please take it as you were taking Yaz. Diane 35 has a different progesterone composition and has the same efficacy as Yaz for the treatment of PCOS. Consult a specialist doctor, discuss with them, and take medications with their consent.
