Hello doctor,
I am a 23-year-old male who began experiencing problems in bed two weeks ago. I began an intensive weight loss (500 to 1000 calorie deficit) a month ago because I had been going to the gym for two years with little to no results. I shed about 15 to 17 pounds in a month and a half with no noticeable difference in my body or lack of strength in the gym. I am usually feeling low because I am self-conscious about my appearance and feel weary because I am going to the gym at least three to four times a week and putting in a lot of work, which seems futile for all the labor and hours I put in. My partner of four years and I agreed to have sex two weeks ago, and while it was hard, my penis softened after five minutes of intercourse. My girlfriend freaked out and started crying, asking why this happened and if I still loved her. It stressed me out because nothing about my feelings for her had changed, and it was the first time this had happened to me, and I have no idea why. Now, whenever she wants to have sex, I am scared and paranoid that it will happen again, and it does. I also have some work-related stress because individuals are being dismissed, and I am not sure if I will be the next to leave. My partner and I discussed it, and we concluded that my weight loss, combined with my body dysmorphia and work-related stress, could be the root cause of my poor libido. I want to get lean and healthy, but I also want to feel normal in bed again. What could be the problem, and how can I cure it? It is affecting my mental health in ways I have never experienced before. Please help.

According to your complaints, I suggest you may be suffering from erectile dysfunction ED
The reasons for erectile dysfunction (ED) are:
Stress due to work or family-related night duties – plan for regular exercise and yoga.
Depression.
Varicocele – ultrasound of the scrotum can help diagnose.
Fear of failure - having one episode of ED once may make you feel you will end up with ED.
Low self-esteem (chronic masturbation).
Homosexuality – not attracted to the opposite sex.
Religious constraints.
Smoking and alcohol.
Long-term illness like cardiac disease, thyroid disease, kidney disease, or any other disease with long-term medication
I suggest you using tablets Zincovit (multivitamin) and Shelcal (Calcium and vitamin D3) for simple vitamin deficiency.
Negative attitude towards your partner.
Hormonal issues like thyroid issues, low testosterone, increased prolactin.
Long-term drug usage.
Diabetes (a disease that occurs when your blood glucose, also called blood sugar, is too high) and Hypertension (a condition characterized by an increase in blood pressure).
Also, I suggest you get the following tests done doppler penis (penile doppler ultrasound is a procedure that is used to predict the erectile muscles), USG (ultrasonography) pelvis, and scrotum, blood pressure, RBS (Random blood sugar), Free T3 (triiodothyronine) and T4 (thyroxine), TSH (thyroid stimulating hormone), and Testosterone (male sex hormone).
Please share more information about the causes listed above so that we can work on diagnosis and therapy.
Also, You need to plan for regular exercise and a proper diet. I suggest you to follow a few instructions mentioned below:
List of exercises: basic exercises like kegels, squats, yoga, running, or fast walking for 5 min.
Food to include: Strawberry, apple, avocado, sweet potato, sesame seed, almond, garlic, and little chocolate.
For a temporary solution, I suggest you can use Tablet. Maildfil (phosphodiesterase (PDE-5) inhibitor) 5 mg twice a day.
