Hello doctor,

I am 30 years old. I am currently experiencing chronic ED and premature ejaculation as well. I have had issues with premature ejaculation more or less my whole life but not as severe as it has become after this sudden erectile dysfunction. I have been to the urologist and he prescribed me with medicine for prostate inflammation, which I took but did not show any noticeable results. I have taken Sildenfil 20 mg and 60 mg with very minimal results. I can maintain an erection at heightened arousal but not for very long and not without stimulation. I was previously drinking daily may be two to four glasses of wine and smoking one to three cigarettes a day both of which I have now refrained from. This dysfunction has been an issue since March and has shown a very little improvement in the five-month period.

Erectile dysfunction and premature ejaculation are usually related problems and must be very annoying problem at your age. From your details, your weight is 160 pounds means you are fit and testosterone is normal. I wonder if you have and long term medical issues like diabetes or hypertension or you are on any long term medications, if so please mention it and send me the list of medications. Otherwise reasons can be unknown in some people. Do you have morning erections? Sudden erectile dysfunction also indicates stress or depression. Are you in a stressful situation or depressed? The medications you took was for prostate which do not think is the issue now. And Sildenafil is short term acting (two to four hours only) and can help only in the erection. Many brands in the market are not original so make sure you buy the genuine product. Second is that it should be taken on an empty stomach. Thirdly, it should be taken 30 minutes to one hour before sex. Erection can only be achieved produced if you get stimulated.

I do not believe I have hypertension or diabetes. I had my blood and testosterone tested and both came back normal. The erectile dysfunction was sudden but has persisted for five months. I have been a little stressed and depressed at certain points but not continuously for five months. I do not have morning erections often and if I do they are not full by any means. Sildenafil was prescribed by a doctor and I believe I took it correctly. As I said, this was sudden and I am well aware what normal erectile function is like and this is far from it.

Thanks a lot for the information given, it helps a lot for the diagnosis. My diagnosis is stress and depression, as this is the only logical explanation for the sudden erectile dysfunction. There is no organic cause per se. in addition to less or temporary action of Sildenafil. All indications are supporting the psychogenic cause of erectile dysfunction. I advise you to get an opinion from the psychiatrist as well for the issue. I will also recommend you to take a longer-acting medicine for erectile dysfunction. Instead of Sildenafil you can take Tadalafil 2.5 mg once daily. If the effect is not enough, you can increase the dose to 5 mg once daily.
