Patient's Query

Hello doctor,

I am 60 years old. I am physically fit and I jog almost 3 to 4 km daily. I am a vegetarian. I have a business of my own. I have undergone angioplasty before 20 years and open heart surgery six years before. But, I am an active person with slim body. My height is 165 cm and weight is 60 kg. My waist is 31 inches.

I have been on medication since my open heart surgery and I was prescribed Olmezest 20, Ecosprin AV 75/20 and Concor 2.5. I am not taking any medicine for the last six months, because I thought that they are affecting my sexual power. But, I find no improvement after stopping the tablets.

My problem is that I am not able to hold myself and enjoy sexual pleasure. Sometimes, I do not get an erection at all and I just ejaculate in a few seconds. I think that I need some medical advice as I still want to enjoy sexual pleasure. I have never tried Viagra.

Please advise what I should do and whom I should consult ?

Hello,

Welcome to icliniq.com. Impotence, characterized by a decrease or loss of the ability to achieve or maintain a firm erection, affects around 20% to 25% of men by the age of 65. Similar to menopause in females, men experience andropause, which can lead to a reduced libido and decreased erection. Various factors contribute to impotence, including heart disease, hypertension, diabetes, and certain medications like anti-hypertensives and anti-diabetics. Heart disease can cause atherosclerosis or thickening of penile blood vessels, leading to reduced blood flow and less effective erections. premature ejaculation. involves ejaculation before a complete erection is achieved. Treating sexual problems requires a thorough understanding of the underlying mechanism and cause, which may include atherosclerosis of penile vessels, decreased testosterone, drug effects, or peripheral neuropathy. Continuing prescribed medications, such as cardioprotective agents and blood thinners, is crucial to maintain heart health. Cardiac testing, along with Doppler of the penis and testosterone level check, should be considered. Impotence may be linked to heart disease and hypertension itself. Consulting with a cardiologist about cardiac health and the use of erectile dysfunction medications like Cialis or Tadalafil and Sildenafil is recommended. Information on nitrates, recent blood pressure, blood sugar levels, lipid profile, and prostate hypertrophy history is important. Physical fitness through yoga and the use of vacuum devices may be beneficial. Avoiding smoking and alcohol, eating antioxidant-rich foods, ensuring sufficient sleep, staying optimistic, and maintaining a healthy workout routine are essential. Never stop medication without a doctor's advice, and if impotence persists after stopping certain drugs, further evaluation is needed.

My recent blood sugar level too normal. I do not have any prostate problem. My cardiologist advised tablet Assurance long back, but I have never taken this. I have also never tried Viagra. I want to get rid of this problem and enjoy sex. I am a bit embarrassed to talk this in front of my doctor.

Hello,

Welcome back to icliniq.com. You need not feel embarrassed before a doctor at all. Please search for a good andrologist in your area and get an appointment. L-arginine is an essential element known for its potential to improve erectile dysfunction or ED. Take it as a tablet or sachet. I suggest L-arginine 2 g per day at a regular basis for six weeks. I also suggest tablet zinc 20mg per day and omega 3 fatty acids one tablet a day. Consult your specialist doctor, discuss with him or her and take the medicine with consent. For your information, L-arginine is found abundant in foods like peanut, soybean, chickpea, pumpkin seeds, lentils and dairy products.
