Patient's Query
Hi doctor,
Does facial fat grafting look unnatural? I am 20 years old and experiencing pretty bad facial fat loss, to the point where any lighting from the side or over makes me look 40. I have decided I want to get fat grafting. Do you have any advice on it?
Thanks.
Hi,
Welcome to icliniq.com.
I can understand your concern.
No, not necessarily. When done by an experienced, conservative plastic surgeon:
It can look very natural, soft, not overfilled.
It restores youthful contours especially mid-face, temples, and under eyes.
Your age (20) works in your favour, as skin elasticity is still good.
The key is:
Microfat grafting or nanofat (smaller, refined fat particles) is used.
Being conservative in volume added — overfilling leads to puffiness or lumpiness.
Having realistic expectations — fat takes months to settle, and 30 to 50 percent of it may get reabsorbed.
From your history, here are the likely contributors:
1. Past weight loss: Even gradual weight loss can reduce facial fat, especially in genetically lean areas like the cheeks and temples.
2. Retinol sensitivity: While Retinol does not cause fat loss directly, some users experience:
Thinner skin appearance.
Tightness and sunken look, especially under the eyes.
Possibly dehydration or irritation, making fat loss look more dramatic.
Retinol likely did not cause the fat loss, but it may have made it more visible by drying the skin.
3. Diet: Eating poorly can also dull your skin tone and reduce collagen support over time. But again, your issue seems more volume-related than texture-related.
I hope this information will help you.
Thanks.
