I recently had female facial feminization surgery. It has been three weeks. I am concerned about possible paralysis of my bottom lip and nerve damage. Is there marginal mandibular nerve permanently damaged?

I saw your photo (attachment removed to protect patient identity) and would admit that there is a slight drop in your left side lip. Anyway, I would like to see a photo of you smiling, in order to come into better conclusions. It could also be an abnormal tensed muscle. I recommend making stretching exercises of your face muscles (also called face yoga) to help reduce muscle tension. Anyway, nerve conduction studies would give more information on possible nerve damage.

In my opinion, this is not a facial nerve palsy. I think that is asymmetry due to a tensed muscle or skin. I recommend a lot of physiotherapy or facial yoga, to help stretch your muscles and skin.
