Hello doctor,

Last year my ultrasound showed grade 1 fatty liver with AST 90 and ALT 143. After one month of diet control, AST and ALT came back to normal and was normal until the next six months. I again got it tested one week back and my ultrasound showing grade 1 fatty liver with 15.7 cm size. My AST is 120 and ALT is 35. Other liver parameters like GGT, ALP are in normal range. I am really scared now, please help me. I showed these to my doctor and he has given me medicines for 20 days. Am I into the serious liver problem?

I can understand your concern. According to your statement, you have been suffering from Grade 1 fatty liver with raised AST (aspartate aminotransferase) level.

According to your investigations result (attachment removed to protect patient identity), your triglyceride level is significantly high, serum TSH (thyroid-stimulating hormone) level is also high but your Vitamin B12 level is below normal.

In case of fatty liver, liver enzymes can be raised. As your serum TSH level is high, so you have subclinical hypothyroidism. For confirmation, you should do a repeat thyroid function test after three to four weeks later. You should take vitamin B12 enriched foods or supplements for your lower vitamin B12 level. For your raised serum triglyceride, you should take Fenofibrate at night and do repeat your lipid profile test after three months later. Avoid all type of oily and fatty foods.

This is really helpful. But please let me know your analysis about my liver. Am I at risk of liver cirrhosis or any such danger?

Deposition of fat in the liver may initiate the inflammation of liver and inflamed liver may turn into cirrhosis of liver. So fatty liver leads to cirrhosis of liver in the long run. As your fatty liver is diagnosed at the initial stage and you are on medications, so I think you are out of danger.

Avoid fatty and oily foods and take Fenofibrate for controlling triglyceride will prevent your liver from any type of further damage. Do repeat the lipid profile with liver function tests after three months and the investigation result will reveal everything.
