Hi doctor,

My father-in-law is about 68 years old. For the last 12-13 days, he is having a fever of 101 to 102. We have shown him to the local physician. As prescribed, he is taking some medicine for fever with some antibiotics. The fever is going down on the effect of the medicine. However, the temperature again goes on this higher side with chills. This situation is not improving. Got his tests done.

I am attaching the prescription and the lab test report. Two to three parameters are especially highlighted, and these are currently under abnormal range. Kindly look into the same and advise on the significance of these parameters and also how much critical the current situation of the patient is?

Just to inform you, he has a long history of diabetes and high blood pressure. He is also taking regular medicine for both of these. He also had a history of right side paralytic stroke about six years ago. Gradually, with medication and constant physiotherapy, he regained his normal (95%) physique.

Kindly look into prescription, lab tests and suggest in detail regarding his current situation. Also, I would oblige you can guide further on the treatment to be done.

I can understand your concern. According to your statement, your father-in-law has been suffering from fever for the last 12-13 days, which is fluctuating in nature.

The prescription and the investigation reports (attachment removed to protect patient identity) that are provided here has revealed that his serum creatinine level is normal within upper limit, otherwise all parameters of his blood reports are almost normal.

He should undergo some investigations like Widal test, CXR (chest x-ray) P/A view, and HRCT (high resolution computed tomography) of the chest as he has a history of cough. Antigen test or antibody test for COVID-19 test or RT-PCR, COVID-19, etc., to detect the underlying cause for his fever and then treat them accordingly.

In the meantime, Cefixime 400 mg twice daily for 14 days should be given to him instead of his current antibiotics.
