Hi doctor,

I am a 62-year-old male who is 5 feet tall and weighs about 175 pounds. I am a vegetarian, I have diabetes for the past 20 years, and my blood pressure is normal. Sometimes my cholesterol is high, and after taking Stanlip 145 mg tablet (one per day), it used to come down, but for the past month, I am not taking it, and in my recent blood test report, it has gone high. Earlier, for many years, I had a Gluconorm-G2 tablet (twice per day), Januvia 100 mg tablet twice daily, and Volix 0.2 mg tablet once daily. I met a doctor last year, and he asked me to change my food habit to low carbohydrates and moderate protein, and high fat; after following his diet schedule, I have reduced 9 pounds and stopped taking Gluconorm-G2 and Volix tablets, and was active till seven months back. After COVID-19, I could not follow the diet, and now when I checked the blood test this month, and found that my blood sugar level and cholesterol level are increased. I also take Clopitab 75 mg and Zincovit tablet 1 per day. I have attached my various blood test reports. Based on these reports, can you please suggest medications?

I have gone through your history and your reports (attachment removed to protect patient identity) that you have attached. I would suggest that your sugar levels are under control and not very high (fasting blood sugar is 135 mg/dL, and post-prandial blood sugar is 189 mg/dL). That is a very decent blood sugar value. I would suggest you continue the tablet Januvia 100 mg twice daily. Regarding your cholesterol, I would suggest you take tablets along with heart-protective tablets. Otherwise, your reports are normal. I suggest you take tablet Januvia (Sitagliptin Phosphate) 100 mg twice daily, capsule Ecosprin-AV (Atorvastatin and Aspirin) 75/10 once daily, and tablet Zincovit (multivitamin) once daily for 30 days. Kindly get back after a month with FBS (fasting blood sugar) and PPBS (post-prandial blood sugar) reports for further follow-up and review.

Hello doctor,

Thank you. I have taken the medicines as suggested by you for the past four months. Again, I have done a blood test, and the test report is attached here. Can you go through the same and suggest if any change is required in the medicine I am taking? My HS-CRP is high as 13.1 mg/L. Please suggest how can I reduce it.

I have gone through the reports you have sent (attachments are hidden to protect the patient's identity). Your sugar levels are under control, so I would suggest you continue the tablet Januvia 100 mg twice in the morning and at night. Regarding your lipid profile, I would give tablets for three months. As you asked, that if you leave cholesterol tablets, the cholesterol levels increase once after stopping tablets that is because you need to follow a strict diet even after stopping tablets, along with physical activity daily for 30 to 45 minutes. If you follow a strict diet and physical activity, your cholesterol levels will not increase. Tablet will help you reduce your higher levels, and it will not help in maintaining your cholesterol levels. So, I need to take tablets for six months below. I am giving you a detailed prescription. Kindly continue the medicine for diabetes continue the same medicine. I suggest you take teh following tablets:

1. Januvia 100 mg twice in the morning and at night.

2. Tablet Ecospirin 75 mg once at night.

3. Tablet Lipikind- F (Atorvastatin and Fenofibrate) once at night for six months.

4. Tablet Zincovit once in the morning.

Kindly check your sugar levels monthly and cholesterol levels every three months. Kindly get back for a consultation next month with sugar tests.
