Hi doctor,

I think I have balanitis. I do have a tight foreskin. I usually have a tear or cut in the foreskin. If I apply Tenovate GN, it goes, but comes again. It is slightly less if I apply a moisturizing cream. I am a diabetic (with medicine PP is 160 to 180). I also had HSV type 1 on genitals also. Please advice. What should I apply?

Kindly send an image and revert back. Without clinical photographs, it is difficult to comment. In the case of a diabetic individual, balanoposthitis is very common. You should not apply Tenovate GN cream (Clobetasol and Neomycin) as it contains a steroid and long-term steroid application can lead to side effects. I suggest using: (Consult a specialist doctor, discuss with him or her and start using the medicines with their consent).

  1. Clotrimazole 1 % cream twice a day.
  2. Fusidic acid 2 % cream twice a day.

If it is associated with white discharge then oral antifungal medications should be considered. But do not apply Tenovate GN cream.

Onabet cream (Sertaconazole 2 %) or DK gel (Miconazole 2 %) or Canesten cream (Clotrimazole 1 %) and Fucidin cream (fusidic acid 2 %). Any one of the above can be used twice daily. Wash area with lukewarm water. Keep the area clean. I still need to see clinical photographs in order to suggest oral medications.

I was a bit reluctant to send the pictures. You understand why, but since you require, I am attaching them hereby. The white deposits are sometimes more while in this picture they are minimal. Putting down the entire summary once again. I also have dryness as you can see in one of the pictures. I am having cuts in the foreskin and white deposits. This is there since many years. The problem reduces a bit if moisturizer is used.

I completely understand your concern and there is no need to worry. These photographs and consultation are private. I have gone through the clinical images (attachment removed to protect patient identity). This is balanoposthitis which is a recurrent problem in diabetic patients. I suggest taking oral antifungal medications, Tablet Fluconazole 150 mg. On alternate days one tablet after a meal for two weeks. Topically apply Onabet cream (Sertaconazole 2 %) or DK gel (Miconazole 2 %) or Canesten cream (Clotrimazole 1 %) and Fucidin cream (Fusidic acid 2 %).

Any one of the above can be used twice daily. Wash the area with lukewarm water. Keep the area clean. If your partner has similar complaints, then even partner management is important.

Hi doctor.

You can see the history here. You had given me Onabet + Fucidin and oral Fluconazole 150 for two weeks, alternate day 1 tablet. For the first month, it was fine. However the same type of cuts re-occurred once I stopped the ointments and now even with Onabet and Fucidin, there is partial relief but not complete relief and the cuts come back. My questions are -

1. What period can I continue Onabet+Fucidin. Are there any side effects?

2. Should I do one more round of Oral Fluconazole 150? How often can this be done.

3. Do you recommend any other treatment.

This recurrence happens if patient is diabetic. Yes you can take a course of fluconazole like last time there is no problem. Also application of topical antifungal for longer duration will not lead to any side effects. But patients do develop resistance with same antifungal cream so you can change to Terbinafine 1% cream (Terbest cream) twice daily or Luliconazole 1% cream once daily.

Same symptoms don't mean you have the same problem. Consult a doctor now!
