Hello doctor,

I have noticed a mild foul smell with signs of infection upon retracting the foreskin (prepuce). I also have mild discomfort and pain. I have had this for a couple of months and used Silver ex cream for a week, but symptoms or infections are recurring. I am suffering from no other issues such as diabetes, urinary tract infection, or any other health issues, and do not have fever. Kindly help.

As per the history provided, it may be candida balanitis. It is a fungal infection of the glans penis under the prepuce. It appears reddish with signs of disease in the form of white discharge, foul smell, and occasional itch and pain. They may develop due to changes in immunity, hygiene issues, changes in hormonal balance, frequent sexual activities, climate changes, and excess usage of harsh chemicals disrupting the mucosal barrier helping to fight infections. I suggest you follow the below-mentioned instructions.

  1. Apply Clotrimazole (Benzyl alcohol) cream on the affected area twice daily for six weeks or till the symptoms reduce.
  2. Wash the affected area with lukewarm salt water twice daily. After cleaning and drying the site, the above cream should be applied.
  3. Take Capsule Becosules Z (Vitamin B complex) once daily after food for four weeks at night.
  4. Take Capsule Itraconazole (Itraconazole coated with starch and sucrose) 200 mg once daily in the morning after food for two weeks.
  5. Avoid using harsh chemicals and soaps or shampoos to clean the area as they may be harmful and disrupt the mucosal protective barrier leading to infections.
  6. Follow a balanced, timely diet, and proper water consumption is advised.
  7. Make sure to inspect your sexual partner, if any, and ensure they undergo treatment to avoid recurrences.
  8. If there is excess secretion, I would advise you to undergo a swab test where the secretion on the affected area is taken for laboratory testing to know the organism that has led to the infection and the medication the organism is most sensitive to.
  9. Avoid sweating and stagnation of moisture in your private parts. Make sure to wear loose clothing and underwear to avoid excess heating and ensure to air your intimate area from time to time to prevent the stagnation of moisture.

Hello doctor,

Thank you for the reply.

I followed the treatment as you prescribed with Clotrimazole and Itraconazole, but symptoms recurred. I have also consulted a gynecologist who put both me and my wife on Doxycycline 100 mg twice daily for 14 days. After completing Doxycycline, for three days I have been noticing small blisters on my glans which are slightly itchy. Kindly suggest.

Since you have been having recurrent infections, I would advise you to follow the below-mentioned instructions.

  1. Undergo a swab test to know the type of organism and the medications the organism is most sensitive to.
  2. Kindly attach an image for assessment of severity. In the absence of an image of the affected area, it is difficult to provide a confirmative diagnosis.
  3. Kindly provide the report of the swab test for confirmatory diagnosis and management plan.
