Hi doctor,

I have a red spot on my nose that would not heal for months. It has not formed a scab. I was worried it might be a broken blood vessel. Is that possible? I do not think it is an ingrown hair because I have looked and do not see one.

I have seen your query and concern and examined your picture (attachment removed to protect the patient's identity). With your descriptions and given picture, it seems you have freckles or nevus.

1. Freckles or ephelis are common in individuals who sunburn easily and do not tan, whereas it is uncommon among people who have dark skin.

2. I suggest 30 % Trichloroacetic acid cautery (TCA) or topical treatment with Depiwhite cream. Applied topically during the night daily for 16 weeks.

3. Follow certain sun protection measures like avoiding mid-day sun, especially 10 AM to 4 PM. Seek shade whenever possible. Wear broad-rimmed hats. Apply Suncros soft gel sunscreen with SPF 50+ daily in the morning all over the face, which can be repeated every 4 hours.
