Hi doctor,

I am a 29-year-old female with height 5 feet 3 inches. I am a black woman. I have been having some severe itchy rashes for one week now that do not resolve with the use of antihistamines. They are red and small papular with clear in color. The issue started last week. I awake with itching and red appearing rashes thought to be an allergy. I could not work due to severe itching, and that was impeding my ability to sleep. I was also diagnosed with COVID-19 (Coronavirus disease 2019) two months back. I have no known drug allergy and no history of prolonged allergic reactions. I have been prescribed Betnovate topical ointment and Zyrtec (Cetirizine) tablets in the nights and Kare (Retinol, Retinyl Palmitate, and Retinyl Acetate, Arnica montana flower extract, and horse chestnut leaf extract) cream diluted with olive oil. Only CBC (complete blood count) reveals no abnormality in the WBC, RBC, and platelet.

Take Teczine-M (Levocetirizine and Montelukast) tablet once daily after food for 7 days. Is that spreading? Do you have any history of fungal infections in the past? Do you have excess sweating? Did you scratch or rub the rashes excessively? Do you have a fever? Answer the questions for details.

Hi doctor,

Thank you for your reply. I have had excess scratching and rubbing since onset, which appears to get worse. I have no sweating, fever nor do I have any history of a fungal illness in the past. The rashes are prevalent on my chest, shoulder and spread along my right arm, legs, buttock, and lower back.

It is an Atopic rash with a fungal infection. Apply KEVON STAY ON (Ketoconazole) lotion 20 minutes before bath for 20 days. KZ (Ketoconazole soap) for a bath for two months. Tablet Loratadine 10 mg once after breakfast for 10 days. Tablet Teczine-M once at night after food for 10 days. Sertaconazole cream (Onabet) twice daily in the morning and night for 3 weeks. After the lesion heals, can use Physiogel moisturizing lotion or Cetaphil moisturizer after bath. Keep the area dry and clean. Avoid sweating. Avoid moisture as it favors fungal growth. Maintain proper hygiene. Wear loose, comfortable thin cotton clothes. Avoid occlusion or friction. Avoid scratching and rubbing of lesions. Keep nails short. Regards.
