I have a black skin mark on my buttocks for the past three years, but now I have inflammation, rashes, and dicomfort. What should I do?

I understand your concern and have seen the attached photos (attachment removed to protect patient identity). From the photos, it looks like an acute on chronic (recent episode on an already present condition) fungal infection / Tinea cruris. There is no need for panic as the active infection will heal within two to four weeks, the black marks caused due to this will heals by its own within six to twelve months. You can take tablet Terbinafine 250 mg once daily at night for two weeks, Terbinafine cream for application once daily at night, tablet Cetirizine 10 mg once daily at night for one week (for itching, it should be continued for two weeks if itching persists), and KZ (Ketoconazole) soap for wash twice daily. Provided there is no prior allergy to these medication and your liver function is normal. Review after two weeks. Do not continue the medications against doctor's advice. Also kindly follow some general measures. Keep the area always dry. Use only dry clothes. Iron your clothes before wearing to kill the fungal molds. Wash area with lukewarm water. Have frequent changing of bed linens and towels. Do not share your personal items. Maintain good genital hygiene. I hope this helps.
