Should one worry if direct bilirubin is over total bilirubin?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 34-year-old male with stomach flare-ups treated with Omeprazole. My liver function test results show total bilirubin, direct bilirubin, and indirect bilirubin within normal ranges. However, since the direct bilirubin is over 20 percent of the total bilirubin (3 out of 7), it might be worth discussing with my doctor to ensure no underlying issues.

Kindly help.

Hello,

Welcome to icliniq.com.

I can understand your concern.

I have reviewed your case history and the attached report. Your symptoms are related to gastritis or dyspepsia and are not linked to liver function issues. The lab results only show bilirubin levels, not liver enzymes like Alanine Aminotransferase, aspartate aminotransferase, gamma-glutamyl transferase, and alkaline Phosphatase.

To manage your symptoms, avoid eating out, reheating meals, and alcohol for a few days, as these can trigger stomach problems. You can take Omeprazole 40 mg, half an hour before breakfast, for two weeks to help with your stomach issues. Additionally, using an antacid syrup like Aluminum hydroxide, 10 mL after meals, and adding probiotics to your diet (either through yogurt or supplements) can be beneficial. If needed, you can check your complete liver function tests and share the results. For your PREP (pre-exposure prophylaxis), consult your doctor and use general precautions like condoms to stay safe.

Please let me know if you need any help.

Thank you.
