Hello doctor,

I have noticed some wart-like thing in my testicle. It has been present for more than eight months now, but still, it is intact. The worrying part is it is not disappearing, it is spread across the testicles. From a young age, I have constantly been facing the issue of warts on my face. Also, I have never had any sexual activity for the last two years. I am wondering about the reason for the same. I use a Zinc tablet 50 mg 2 times daily to cure my erectile dysfunction. To remove the wart on my face, I use salicylic acid. Would you please suggest a good treatment for my wart? Thanks.

I have seen the attached image (attachments removed to protect the patient’s identity), which looks like warts. Have you ever done a biopsy of your lesion? As around 50 percent of warts are associated with HPV (human papillomavirus) infection, you should get tested for HPV. Kindly keep the area dry and clean. Wear cotton underwear to avoid moisture. Use Imiquimod 5 % cream topically. If it does not heal or keeps recurring, then cryosurgery removal or surgical excision may be necessary.

Persistent HPV infection can lead to squamous cell carcinoma. Hence the testing is recommended. You can use Aldara 5 % (Imiquimod) cream thrice daily for one week.

I have seen the attached reports (attachments are hidden to protect the patient's identity). It is pretty clear that the PCR (polymerase chain reaction) test is negative for HPV infection. What is the current severity of your warts?

Thanks again. As prescribed by one of the doctors here, I applied a lotion to the warts, and in two weeks, 90 % of the warts disappeared. But the problem is it came back. Is anything there I should be worried about, like cancer or something? Also, please let me know if my partner will be affected by this, as I am married. What are the precautions I should take? Please suggest the medication.

I understand your concern.

Certain conditions like weak host immunity cause warts to come back. A change of treatment regimen may be required. You should try Sinecatechins 15 % ointment three times a day, for up to four months. It has antioxidant, anti-inflammatory, and anti-tumor properties. If improvement is not seen after a week, then treatment should be stopped, and an alternative regimen will be started. Also to mention, if the PCR report is negative, it is not necessary that HPV is not present. PCR has 96 % sensitivity. A gene sequencing may be required to detect a strain that a PCR test can possibly miss. PCR can miss the presence of viral variants, distantly related HPV types, or low viral loads. Yes, you need to protect your partner, as warts can be highly contagious. I think there is no need to worry about anything like cancer as of now.

The following measures can be taken to minimize risk to your partner:

1. Ensure she has taken all doses of the HPV vaccine.

2. Use a condom every time you have sex. This will reduce the chances of spread but will not eliminate it. The ideal way is to avoid sexual contact till you get treated. But this is rarely feasible.

3. Get your partner tested with a pap smear and a PCR test.

4. Keep your warty area dry.

5. Do not scratch your warts.

Warts may take months and even years to go away completely. You need to be patient, stick with treatment, and even change treatment whenever necessary.
