Hi doctor,

I was operated on for nasal polyps surgery three years back. For the last one month or so, it seems that some mucus is always stuck in my throat, and I have to clear it every now and then. This gets worse while lying on the bed. I showed it to my ENT specialist, and he mentioned that it is GERD and prescribed some antacid tablets that did not work. I have no other symptom, and I always feel some phlegm in my throat that hardly comes out or goes inside while swallowing. This is very irritating. Please prescribe some medicine or any lifestyle changes to get rid of this.

I reviewed your history and the ENT (ear nose throat) Surgeon's attached file (attachments removed to protect the patient's identity).

The problem you described is very typical of two things

1) Postnasal drip.

2) GERD (gastroesophageal reflux disease).

As ENT Surgeon has seen you, and they ruled out postnasal drip, so it is more in favor of GERD, and another supporting evidence is its aggravation on lying down.

I will recommend starting the following treatment and then get back to me after two weeks and let me know the response. Antacids will not help in it.

Differential diagnosis

GERD.

Postnasal drip.

Probable diagnosis

GERD.

Treatment plan

1. Capsule. Dexlansoprazole 60 mg one before breakfast for eight weeks. 2. Tablet Cetirizine 10 mg one at night for two weeks. 3.Tablet Levosulpiride 25 mg 1+1+1 before meals for ten days.

Preventive measures

1.Try to take dinner at least two hours before going to bed. 2. Elevate head end of bed about 5 inch. 3. Avoid smoking and alcohol. 4. Avoid evening coffee and tea.

Regarding follow up

Let me know after two weeks.
