Hello doctor,

There is optic disc asymmetry 0.5 and 0.7. Normal vision is 6 by 5. I am 30 years old male. Normal BP. I have a complaint of severe migraine like headache. When the headache starts, there is swelling around the eye. What can this asymmetry be due to? Can this be glaucoma?

I have gone through what you have mentioned. There are several causes that cause an asymmetry of optic cup-to-disc ratio, but glaucoma is the main disease that we try to rule out as it sometimes manifests without any pain and hence can go undetected. Migraine headaches are due to blood flow abnormalities in the region of the face and inside the cranium (skull bone). Hence, there is a chance that this eyelid swelling could be due to a migraine. My advice to you is since you have been detected to have optic disc asymmetry, kindly follow up with your ophthalmologist every six months or as instructed by him or her. You may also be offered additional testing to confirm if your disc asymmetry needs to be treated or if it should just be monitored. Continue with your anti-migraine medication and watch for any changes in your eyelid puffiness. In case there are changes such as longer duration or if you have an itchy sensation or feel like rubbing your eyelid, then please bring it to the notice of your ophthalmologist.
