Hello doctor,

I am suffering from extreme hair fall and boils on my scalp. Hair is falling from the root for the last couple of years. These boils come with pus, and they settle down, become dry and then turn into dandruff. It is quite painful when the boils are in full swing and feels as if someone is pulling my hair. I have had this problem earlier and I have seen many doctors. They have loaded me with medicines and supplements. Every time I consulted a doctor, I would have their medicines for a month or two and then stop the medicines thinking that it has not suited me or that I could find no difference.

They use terms that confuse me more than giving me confidence. I tried allopathy, Unani, and yoga for blood circulation. One doctor even suggested mesotherapy, serums, etc. But now the hair fall is so bad that I have switched from a normal-sized clutch to using a clutch that is two sizes smaller and using even that pokes me. There is hair everywhere when I shampoo, comb, or if I just run my hands through it. There are a lot of gaps. Literally, it pulls down my confidence. I am currently taking the tablets Revital Woman once daily, BTN Forte once daily, and D-rise sachet once a week with milk. Previously, I was taking the tablets Armet Plus, Tricofol, and many other multivitamins. I also used Minoxidil serum long back. Please help.

I can understand your concern.

I went through all your pictures (attachments removed to protect the patient's identity), investigation, and detailed history. Recurrent boils on the scalp can be a continuous problem for many people. It can also be a pre-runner for seborrheic dermatitis. Minoxidil should not be used irregularly. Once started with Minoxidil, the result will be obvious only after six months and you should not stop in the middle. Otherwise, it will be followed by severe hair fall. Your vitamin D level is low, but you are already on medication. Hemoglobin is also very low.

Please start taking iron supplements along with fresh vegetables, dates, and fruits rich in iron. Keep your dandruff under control. Continue taking the multivitamin tablets BTN Forte (Biotin forte) and Revital. For iron, take the supplement Zincofer tablet daily. I suggest you start taking the capsule Microdox-LBX (Doxycycline and Lactic acid bacillus)100 mg once a day for two weeks. Do not scratch the area. Use Ketoconazole and Zinc pyrithione (Scalpe) shampoo. Dilute it with water, and make sure that there is at least 10 minutes of contact with the scalp. Then wash it off. Second-line antibiotics are to be changed only step by step.

Yes, all medicines have to be taken after food only. It will take a minimum of three months to get your hair fall under control. Tablets will not stop the hairs that are already affected and falling now. It is only to induce new hair growth in the area of hair fall. Raise and maintain your hemoglobin level above 12.

I am done with taking the tablet Microdox-LBX 100 mg for two weeks. What should I do now? Other medications include Zincofer, BTN Forte, and Revital. The boils are better but have not completely gone. Kindly suggest further treatment plans.

Please continue taking the other tablets for two months continuously. Also, take iron-rich foods and vegetables. I suggest you take the tablet Ambroxol and Loratadine (Alaspan) once a day for four weeks starting today. Do not scratch the area. Give rest to the scalp and wash your hair on alternative days. Continue using Ketoconazole and Zinc pyrithione (Scalpe) shampoo. After two weeks, give rest for the tablet Microdox-LBX (Doxycycline and Lactic acid bacillus- two weeks from now). Start taking the tablet Minoz Od 50 or MI 50 (Minocycline) once a day for two weeks. This antibiotic works in a different way to control your boils. Kindly get back for review after one month.

I have the following doubts:

  1. Alaspan should be taken once a day for four weeks. Am I right?
  2. Minoz Od 50 mg or MI 50 mg should be taken once a day for two weeks. Am I right?
  3. Is Minoz plain also fine as only that is available?
  4. Should Microdox-LBX be taken for two weeks from now or should I give rest for two weeks and then take it for two weeks?

You have finished the Microdox-LBX course. So, give rest for two weeks. Then, start taking the tablet Minocycline (Minoz). Minoz plain is fine and you can take it for two weeks. Start taking a tablet of Ambroxol and Loratadine (Alaspan) from today once at night.

I have completed taking the tablets Minoz and Alaspan and I am done with my one-month course. At the moment, I am taking the tablets Zincofer, Revital, and BTN Forte 10 mg. There are no boils now but hairfall is there. What is the next course of medicine? Kindly suggest.

Please give rest. All the hairs affected by boils will fall. New hair will have to grow in that area. I suggest you continue taking the hair supplements along with BTN Forte and Zincofer for three months. Also, do the following:

  1. Keep your body cool.
  2. Drink lots of water.
  3. Take fresh vegetables and fruits rich in iron.
  4. Keep dandruff under control.
  5. Use shampoo on alternative days.
