Hello doctor,

I am enquiring about a patient who underwent heart surgery around 13 years ago. Angiography was conducted, and angioplasty was performed. Currently, he takes the following medications Ecosprin Gold 10, Glycomet 250, and Etizola 0.5. He used to take Vomistop, DDR-D, Domperidone, and Pan-D capsules. He got an LFT done, and all the results were normal. Please suggest the best medicine that will not cause side effects to the liver, heart, and elsewhere. This problem has been going on for the last fortnight.

I have read your concern and will help you out. But first, I would like to know a few things.

1) What are the current symptoms the patient is having?

2) What is the age of the patient?

3) Is the patient a smoker?

4) What is the blood pressure and heart rate?

For now, these medicines are fine. They only have a mild effect on the liver and kidney. But to advise a better group of medicines, I will need the answer to these questions. So please respond with answers to these questions so that I can set the medication for the patient, and they can continue these medicines for a long period without any side effects on any major organs of the body.

Hello doctor,

The current symptoms are constant nausea, loss of appetite, dizziness, feeling of lack of strength in the body, palpitations, and tingling. The blood pressure is 113/83 mm Hg, and the heart rate is 80 beats per minute. The patient is not a smoker and is 58 years old.

The symptoms you have mentioned can be a sign of acute heart failure. Does your patient have swelling in the legs or face? Does he have pedal edema? I will advise you to get a 2D Echocardiography to look for the exact cause and find out the EF (ejection fraction) of the heart.

I also advise you to consult a specialist and take the following medications with their consent which will help your patient in the long run.

1) Capsule Ecosprin Gold 20 (Aspirin, Atorvastatin, and Clopidogrel) at night.

2) Tablet Met XL 25 (Metoprolol Succinate) once daily.

3) Tablet Melmet 250 (Metformin) twice daily.

4) Tablet Telmikind 20 (Telmisartan) once daily.

5) Tablet Cyra-D (Domperidone and Rabeprazole) once in the morning.

6) Tablet Dytor Plus (Spironolactone and Torasemide) once in the morning.

If the symptoms persist, you should also get kidney function tests, as increased creatinine can cause these symptoms. In addition, the diet should be in smaller portions and frequently taken. I hope this helps. Kindly revert in case of further queries.
