Hello doctor,

I am 18 years old and have been experiencing extremely heavy periods for a year. For the past six months, I have been battling bleeding through clothes, clots of blood during my period, and even signs of anemia. The bleeding is so severe that I have to change the sanitary pads every one or two hours. I am experiencing blackouts, dizziness, painful cramps and pale lips and skin. I have consulted a doctor, who has recommended me an ultrasound and some blood tests. I wanted to know if there is a disease like menorrhagia or endometriosis and if there is any natural remedy to treat this problem.

Please provide me some answer.

I have read your query and understand your concern.

You are young and may experience irregular and anovulatory cycles as your reproductive system is still underdeveloped. The symptoms like dizziness and weakness are due to low hemoglobin levels caused due to heavy periods. I suggest you carry out all the blood tests your doctor has prescribed. I will suggest you undergo the following tests -

  1. Ultrasound scan of the pelvis (per abdominal with a full bladder if not sexually active, or transvaginal if sexually active).
  2. Complete blood count (CBC).
  3. Thyroid function tests.

I suggest you take the following medications and follow the instructions mentioned below -

  1. Tablet Ponstan (Mefenamic Acid) and tablet Tranexamic acid (antifibrinolytic agent) 500 mg every eight hours from the second to the fifth day of your period to reduce bleeding.
  2. Tablet Norethisterone (synthetic progestin) 5 mg twice daily from the fourth to 24th day of every cycle for three consecutive cycles. This will give hormonal support and regulate your menstrual cycles.
  3. In case of severely reduced hemoglobin levels, you will require a blood transfusion.
  4. Have iron supplements to prevent anemia.
  5. A healthy diet, including carrots and apples, as they help increase hemoglobin.
  6. Exercise regularly and indulge in healthy recreational activities.
  7. Please send your scans and blood tests reports to understand your case better.
