Patient's Query

Hello doctor,

I have no known comorbidities. My height is 6 feet, and I weigh 174 lbs. I have generally been active in physical exercises. Two months back I started having a dull constant stomach ache in my upper central abdomen. Since then, my symptoms include not feeling too hungry, burping more than usual, increase in pain in the central abdomen after eating. I stopped eating wheat, dairy, spicy products, etc., for two months. I took alcohol only as a social drinker and stopped that for two months. I have even switched to bottled water for the last 15 days.

Initially, I took homeopathic medicine for a month for a possible ulcer, with no relief. Then I consulted a general physician who gave me table Omeprazole and some anti-spasmodic medicine which I took for a few days. However, the dull pain continued. I underwent abdomen and prostate ultrasounds and blood or urine tests. The tests came normal. I was told I might have gastritis and was given a proton pump inhibitor again, which I took for seven more days.

I then underwent a blood test IgA for Helicobacter pylori on my own, which came positive (52.81 U/ml). I also underwent a separate urine culture test for urinary tract infection on the advice of a physician, which also came positive for Citrobacter koseri bacteria. When I referred these results to my general physician, the doctor said there was no requirement to take any separate medicine for H.pylori and added a tablet Cipla (Cyproheptadine) 500 mg for seven days in my medicine for urinary tract and asked me to continue tablet Pantoprazole (proton pump inhibitor) for gastritis symptoms. I have taken tablet Pantoprazole for the last eight to 10 (empty stomach in the morning) and tablet Cipla 500 (BD) for two days. However, my dull pain and symptoms remain the same.

My queries are as below:-

  1. Is there no requirement for any different treatment for H.pylori?
  2. Is tablet Cipla 500 mg, which I am taking for UTI (which I think might be a separate problem from stomach pain), also effective against H.pylori infection?
  3. Is there a need for taking some other antibiotic in addition to H.pylori ( I understand that H.pylori antibodies do not refer to a current infection. But may I also inform you that I have never earlier taken any antibiotics for seven days or more, especially for H.pylori)? If yes, what?
  4. Why may I not be getting any relief?
  5. Is there a requirement for me to immediately undergo upper gastrointestinal endoscopy or CT scan since the symptoms persist for two months?

I will be grateful if you can help me because I am very worried since, despite these medicines, my symptoms remain the same.

I understand your concern.

I have thoroughly reviewed your case history. There is a specific treatment for h.pylori dual antibiotics; for 14 days, the tablet Ciprofloxacin is not included in the H.pylori treatment regimen. For h.pylori treatment, I mentioned two antibiotics simultaneously for two weeks. Get an H.pylori stool antigen or urea breath test to treat h.pylori. I would not prefer treatment on a blood test for h.pylori. It would help if you visited a gastroenterologist for proper history and physical examination, do not rush for an endoscopy or CT scan immediately.

Certain alarming symptoms should be noticed, like blood in vomiting or stools, unintentional weight loss, family history of tuberculosis or cancer, or any lump feelings in the stomach. If all of the above are negative or positive, I recommend visiting a gastroenterologist. He will decide on further investigations depending on your history and dirt.

Thank you for the reply.

Do I need to take antibiotics for H Pylori? Since the doctor said that the tablet Pantoprazole is good enough and there is no requirement for antibiotics for H Pylori. Please clarify the need to take antibiotics for H Pylori.

Treatment for h.pylori with dual antibiotics for two weeks is not recommended based on the blood h.pylori test if you want to know whether you are suffering from h.pylori infection right now, yes or no.

Go for either the h.pylori stool antigen test or urea breath test. If any of these will come positive, then you need treatment.
