Hello doctor,

I have two skin tags near the anus. It pains a lot while passing stools, and even after that, it remains for a few minutes. It is there for about a year. Initially, I do not have any problem, and I am getting this pain only from last month, which is very severe for a week now. Kindly help.

Well, these are not anal tags. They are thrombosed external hemorrhoids, which are often painful. It may or may not associate with constipation. But yes, it is a risk factor for their development.

The definite treatment is surgery if it is painful. I suggest you start the following regimen.

Take tablet Metronidazole 400 mg (antibiotic) thrice daily for five days, tablet Daflon 500 mg(diosmin and hesperidin) twice daily for one week, and take Ispagol husk (dietary fiber) two tablespoons full with a glass of water, irrespective of whether you have constipation or not.

Apply Diltiazem (calcium channel blocker) gel with the help of nozzle four times daily six hours apart. If the pain still does not settle, use Xylocaine or Lidocaine gel 2 % use with nozzle four times daily until the pain resolves.

Do a sitz bath. Take semi warm water and fill in the tub to half. Add 5 ml of Dettol antiseptic and sit in it at least three times daily for now. Then you can decrease the frequency once the pain gets settled.

Let me know how you are doing in the next 48 to 72 hours. I would add something more to it if the above regimen does not help you.
